Adult eye tests are for customers who are 16 and above.
If you have a pre-existing eye or medical condition, please add a note at the bottom of this form.

If you would like to book a number of adjacent appointments for more than one person, such as an "eye test aged 15 and under" and an "eye test aged 16 and over", please phone the store.

We advise that children should have their first eye examination at around three years old.
A child eye test at Specsavers is a tailored eye test adapted to your child's age, capabilities and expected levels of development and growth
A parent or guardian must accompany children aged 15 or under.

During your contact lens assessment / trial, our professional contact lens consultant will examine the health of your eyes and help you decide which lens is right for you.
Please allow up to one hour, as this may also include a session with one of our contact lens team who will guide you on how to apply, remove and handle contact lenses safely and with confidence.
In order to book this appointment a valid sight test prescription is required.
A valid prescription normally lasts for 2 years, an expiry date will be recorded on your last prescription card. If you need a valid prescription, please request a sight test prior to this assessment.
